Meeting people “where they are at.” The Rural Community Health Worker Network was founded in April 2018 with a mission focused on collaboratively building a national system focused on the well-being of the individual and the community. Whether it is our network communities or The C.O.R.E. Project community; Community Health Workers serve as culturally competent liaisons to connect individuals to vital resources that stabilize overall well-being.
